Job Description
職責：
- 根據社署安老服務統一評估機制，到內地為申請廣東院舍照顧服務計劃的香港長者進行評估
- 負責個案管理，並到廣東院舍照顧服務計劃之認可院舍為香港長者提供關顧服務
- 協助宣傳、行政、財務等事宜
- 配合機構跨境服務規劃發展方向，建立地區網絡
- 按需要到內地不同城市開展服務
要求：
- 持香港認可社會工作文憑或以上之註冊社工
- 具安老服務統一評估機制之認可評估員資格
- 具1年或以上社會服務經驗，有安老服務經驗優先
- 具備良好的中英文書寫能力，流利粵語
- 良好溝通能力，能獨立處事及出差，個性熱情，具責任感
- 有與內地接觸的經驗優先，熟悉社工服務、社會政策優先
- 需往返香港與內地工作
工作地點：大圍
44小時，視乎情況，須週六日工作
