Social Worker 2 (Ref: SW2_DHCE-ST_NOV24)

• Registered Social Worker with Bachelor Degree or above in Social Work
• 3-5 years’ work experience in community education and health related
• Mature, independent, good interpersonal skills and teamwork

Key Responsibilities:

• Develop and implement patient empowerment programs, psychosocial programs, mutual support services and providing individual counselling to enhance client’s self-management and better problem-solving skills in face of potential health risks
• Network with different community partners to organize and execute the health promotion services in order to facilitate client’s easy access of community resources to cope with their health challenges
• Plan and develop community and patient volunteer scheme in supporting the services and to create more alliances to foster a caring community

Requirements:

• Have a Certificate of Registration and a valid Practicing certificate issued by Social Workers Registration Board, Hong Kong
• With Bachelor Degree or above in Social Work
• 3-5 years’ work experience in community education and health related field
• Good command in written and spoken Cantonese, English and Mandarin
• Mature, independent, good interpersonal skills, teamwork and use of technology in service delivery
• Duties in evenings, weekends and public holiday are required on need basis
